I hereby request Sawakami Asset Management (Thailand) Company Limited to or not to deliver and disclose any information in connection with my investment in the Tax-Exempted Mutual Funds throughout the tax year beginning from the tax year of the date I submit this request to the Revenue Department for the benefit of my personal income tax exemption in accordance with the regulations, conditions and procedures specified by the Revenue Department.
